#### 7 Duty of disclosure—written notice
(1) This section is made for the purposes of paragraph 22(4)(a) of the Act.
(2) The form of writing that may be used to inform an insured of the matters mentioned in subsection 22(1) of the Act is:
(a) for a contract of general insurance—the form set out in Part 1 of Schedule 1; and
(b) for a contract of life insurance—the form set out in Part 2 of Schedule 1.
(3) The form of writing that may be used to inform another person who may become a life insured (other than under a consumer insurance contract) of the matters mentioned in subsection 22(1) of the Act is set out in Schedule 2.
> Note: This section does not apply to consumer insurance contracts (see section 20E of the Act).
