#### 67W Information to be provided if debt is referred to Chief Commissioner 67W Information to be provided if debt is referred to Chief Commissioner > > (1) If the Health Secretary refers a matter to the Chief Commissioner for debt recovery action under the [State Debt Recovery Act 2018](/view/html/inforce/current/act-2018-011), the Health Secretary must provide to the Chief Commissioner the following information for the purposes of enabling the Chief Commissioner to exercise functions under that Act— > > > > > (a) identifying information about the person who was provided with the ambulance services and, if that person was a child, about any parent or guardian of the child, > > > > > (b) the date the ambulance services were provided, > > > > > (c) the time the ambulance services were provided, > > > > > (d) the location or pick up address at which ambulance services were provided, > > > > > (e) the destination to which the person was taken after pick up, > > > > > (f) the distance travelled as part of the ambulance services, > > > > > (g) the unique identifying number allocated to the ambulance services by the Health Secretary, > > > > > (h) any other information of a kind prescribed by the regulations. > > > (2) A requirement to provide information is a requirement to provide so much of that information as is known to the Health Secretary. > > > (3) This section applies in addition to any requirement to provide information under the [State Debt Recovery Act 2018](/view/html/inforce/current/act-2018-011). > > > (4) In this section— > > > > identifying information has the meaning given by the [State Debt Recovery Act 2018](/view/html/inforce/current/act-2018-011). > > **s 67W:** Ins 2014 No 60, Sch 1 \[4\].
