What it does
This Determination, made under subsection 3C(1) of the Health Insurance Act 1973, creates two Medicare Benefits Schedule items for heart health assessments that would not otherwise exist in the general medical services table. It specifies a new item 699 for attendances by a general practitioner and a new item 177 for attendances by a prescribed medical practitioner (as defined in the General Medical Services Table Regulations). Section 6 of the Determination provides that a relevant service provided in accordance with this Determination and as a clinically relevant service is to be treated, for all relevant provisions of the Health Insurance Act, the National Health Act and their regulations, as if it were both a professional service and a medical service, and as if there were an item in the general medical services table that related to the service and that specified the fee set out in the Schedule. This mechanical effect is critical: without section 6, these services would not attract Medicare benefits. The Schedule sets out two items. Item 699 carries a fee of $80.10 for a professional attendance by a general practitioner at consulting rooms, lasting at least 20 minutes, and including collection of patient history, a basic physical examination that must record blood pressure and cholesterol, initiating interventions and referrals as indicated, implementing a management plan, and providing preventative health care advice. Item 177 carries a fee of $64.10 for the same service components when performed by a prescribed medical practitioner (a medical practitioner who is not a GP, not a specialist, and not a consultant physician, as defined in clause 7.1.1 of the General Medical Services Table Regulations). The Determination includes a frequency limitation in section 7: item 699 or 177 is not applicable to a particular patient if the patient has been provided that particular item in the previous 12 months, or if the patient has been provided any item in Group A14 of the general medical services table (excluding item 715) or any item in Group A7 Subgroup 5 of the General Medical Services Table Regulations (excluding item 228) within the previous 12 months. This means a patient who has had a health assessment under certain other MBS items cannot receive a heart health assessment under this Determination within that year. The Determination also includes a one-off indexation provision in section 8: at the start of 1 July 2025, each fee amount is replaced by 1.024 times the amount immediately before indexation, rounded to the nearest five cents. The Determination is repealed on 30 June 2028 at 11:59 pm, meaning these items will cease to exist after that date unless a new instrument is made.
