What it does
This Determination adds two Medicare items for extracorporeal photopheresis (ECP) for chronic graft versus host disease (chronic GVHD) to the general medical services table under the Health Insurance Act 1973 framework. The instrument is made under subsection 3C(1) of the Act (s 3 Authority) and, by operation of s 5, treats a relevant service specified in the Schedule as both a professional service and a medical service for the “relevant provisions” of the Act and associated regulations. The Schedule inserts Group T1 Subgroup 8 Haematology items 13761 and 13762 (Schedule). Each item is payable “once per treatment session (H)” at the fee stated in Column 3: $1,908.35 (Schedule). The Determination defines a treatment cycle as treatment beginning on a day and ending 12 weeks after that day, and a treatment session as a single ECP treatment in a series of sessions that make up a cycle (s 7).
The items are subject to specific clinical and delivery preconditions set out in the Schedule. Item 13761 applies to a person who has had allogeneic haematopoietic stem cell transplantation, developed chronic GVHD, and for whom steroid treatment is clinically unsuitable because the disease is steroid refractory, steroid dependent or steroid intolerant; the person must not have previously received ECP (Schedule, item 13761). Item 13762 applies where a person has previously received an ECP treatment cycle and achieved a partial or complete response in at least one organ, and further ECP is required (Schedule, item 13762). Both items require delivery using an integrated, closed ECP system, provision in combination with methoxsalen that is listed on the Pharmaceutical Benefits Scheme, and that the service be provided by or on behalf of a specialist or consultant physician practising haematology or oncology with experience in allogeneic bone marrow transplantation (Schedule).
