#### 78 Radiation Oncology Register—other information to be included in application for registration For the purposes of paragraph 23DZZO(1)(d) of the Act, the other information that must be included in an application for registration of radiation oncology premises or a base for mobile radiation oncology equipment is the following: (a) the nature of the practice at the premises or base; (b) for radiation oncology premises—the quantity of each type of radiation oncology equipment ordinarily located at the premises; (c) for each piece of radiation oncology equipment ordinarily located at the premises or base that has a serial number or other identifying number—that number; (d) the functionality of each piece of radiation oncology equipment ordinarily located at the premises or base. > Note: Example: For paragraph (a), the practice might be a base for mobile equipment, a specialist radiation oncology practice (either on a stand‑alone practice site or co‑located with a primary care practice or group), a primary care practice, a sports medicine clinic or a public hospital.
