58B Meaning of case conference team in item 880 (1) For item 880, a case conference team: (a) includes a specialist, or consultant physician, in the practice of his or her specialty of geriatric or rehabilitation medicine; and (b) includes at least 2 other allied health professionals, each of whom provides a different kind of care or service to the patient and is not a medical practitioner or family carer of the patient; and (c) may include the patient, a family carer of the patient or a medical practitioner.
