3 Revocations
All previous approvals made under subsection 23DB(1) of the Health Insurance Act 1973 are revoked.
4 Approval of forms of undertaking (Act s 23DB(1))
(1) The form contained in Schedule 1 is the approved form of undertaking to be
given by persons who wish to become approved pathology practitioners.
(2) The form contained in Schedule 2 is the approved form of undertaking to
be given by persons who wish to become approved pathology authorities.
