41 Surrender by holder (1) The holder of a licence, permit or certificate of registration may, by written notice to the Commissioner accompanied by: (a) the licence, permit or certificate; or (b) a statutory declaration setting out the reasons why the licence, permit or certificate is not accompanying the notice, surrender the licence, permit or certificate.
