30 Filing or issuing of documents—forms
A document filed in or issued from a court in proceedings under section 111CN or 111CW of the Act, or regulation 15 or 26, must contain a heading in accordance with Form 1 and be in the following form: (a) for an affidavit—Form 2; (b) for an application for an order—Form 3; (c) for a notice of an application for an order—Form 4; (d) for a response to an application for an order—Form 5; (e) for a reply in response to an application for an order—Form 6.
