82 Application for ballot—prescribed form (s 94(4))

An application for a ballot under section 94 of the Act must: (a) be in accordance with Form 2; and (b) contain the information prescribed in that form; and (c) nominate a person to be the representative constituent member for the ballot to receive documents on behalf of the applicant and for any other purpose specified in this Division.
