#### 155 Application for revocation of certificate (s 249(4)(a)) For paragraph 249(4)(a) of the Act, an application must be lodged with the FWC, together with the following: (a) a declaration signed by the secretary or an officer of the organisation authorised to sign the declaration verifying the facts set out in the application and in the documents being lodged with the application; (b) details of the existing reporting units; (c) a copy of any resolutions concerning the proposed division made by the respective committees of management of the organisation and of each branch affected by the proposed division; (d) a statement explaining how the level of financial information that would be available to members of a reporting unit under the proposed division would be adequate and relevant.
