21 Making application (1) An application for a CDS approval must be: (a) made in the approved form to the CEO; and (b) accompanied by: (i) a copy of each waste management arrangement relevant to the approval; and (ii) in relation to an application for a coordinator approval – a copy of the agreements mentioned in section 12(3) entered into by the parties to the agreement; and (iii) any additional information to enable the CEO to decide the application. (3) If the application is for a collection approval, the application must state the days and times during which it is proposed to open the collection depot to the public.
