### 29 Records and notifications of determination of first request > > (1)  A person’s PMP who has made a determination under [section 26](#GS26@EN) in relation to the person must, as soon as reasonably practicable but in any case within 7 days – > > > > > > (a) notify the person as to whether the PMP’s determination states that the person is, or is not, eligible to access voluntary assisted dying; and > > > > > > > (b) place, on the PMP’s medical records in relation to the person, the determination or a copy of the determination; and > > > > > > > (c) give to the Commission a copy of the determination and a statement, in the approved form, setting out the PMP’s reasons for the determination. > > > (2)  A person’s PMP who has made a determination under [section 26](#GS26@EN) in relation to the person may, at the request of the person, provide to the medical practitioner who the person ordinarily attends in relation to a disease, illness, injury, or medical condition – > > > > > > (a) a copy of the PMP’s determination; and > > > > > > > (b) a statement of the reasons for the PMP’s determination.
