sec.147 Information that must be accessible by providers— Act , s 62

For section 62 (3) (a) , (5) and (6) of the Act , the following information is prescribed— the person’s full name and any previous family names of the person; the person’s sex; the person’s date of birth; the account number for the person’s student account.

(a) the person’s full name and any previous family names of the person;
(b) the person’s sex;
(c) the person’s date of birth;
(d) the account number for the person’s student account.
