#### 19 Application for voluntary suspension of provider approval 19 Application for voluntary suspension of provider approval > An application under section 37 of the Law for the suspension of a provider approval must include the following information— > > > (a) the full name of the approved provider; > > > (b) the provider approval number; > > > (c) the name and contact details of the contact person for the purposes of the application; > > > (d) the reasons for the suspension; > > > (e) the date on which the suspension is proposed to take effect and the duration of the suspension; > > > (f) what is intended to happen to each education and care service operated by the approved provider during the proposed suspension; > > > (g) a statement indicating that the approved provider has notified the parents of children enrolled at the education and care services operated by the approved provider in accordance with section 37(3) of the Law.
