#### 17 Application for amendment of provider approval 17 Application for amendment of provider approval > An application under section 22 of the Law for an amendment of a provider approval must include the following information— > > > (a) the full name of the approved provider; > > > (b) the provider approval number; > > > (c) the name and contact details of the contact person for the purposes of the application; > > > (d) the details of the amendment applied for; > > > (e) sufficient information or documentation to support the application for amendment.
