sec.44 Form of application for service approval
The application for a service approval must be in the approved form and accompanied by any fee prescribed under a regulation. The application may be made by more than one person. If an application is made by more than one person— the information required must be provided for each person; and the requirements of this division must be complied with by each person. The applicant also must provide any other relevant information reasonably required by the chief executive under section 46 . s 44 amd 2025 No. 24 s 39R (sec.44-ssec.1) The application for a service approval must be in the approved form and accompanied by any fee prescribed under a regulation. (sec.44-ssec.2) The application may be made by more than one person. (sec.44-ssec.3) If an application is made by more than one person— the information required must be provided for each person; and the requirements of this division must be complied with by each person. (sec.44-ssec.4) The applicant also must provide any other relevant information reasonably required by the chief executive under section 46 .
- (a) the information required must be provided for each person; and
