12B Exemption certificate—form An exemption certificate issued for a child must— (a) state the day it is issued; and (b) state the child’s name; and (c) state the period for which it is in force; and (d) for an exemption from the full-time participation requirement— state the details of the exemption; and (e) state any condition to which it is subject; and (f) contain any other particulars prescribed by regulation.
