sec.68 Form of application

A disability worker screening application must be— made in the approved form and in an approved way; and signed by the applicant; and accompanied by the fee prescribed by regulation for the application. (sec.68-ssec.2) The approved form must provide for the applicant— to provide proof of the applicant’s identity; and to consent to being screened under this part; and if the applicant is engaged, or proposes to be engaged, to carry out disability work—to provide information about the entity that has engaged, or proposes to engage, the applicant to carry out the disability work.

• (a) made in the approved form and in an approved way; and
• (b) signed by the applicant; and
• (c) accompanied by the fee prescribed by regulation for the application.
