147 Reporting emergency use of regulated restrictive practices—disability service providers (1) The Authorised Program Officer for a disability service provider must give to the Senior Practitioner a monthly report on the use of a regulated restrictive practice under section 145. (2) A report under subsection (1) must be given within 7 days after the end of each month and specify— (a) the form of the regulated restrictive practice used and, if seclusion was used, the period of time during which it was used; and (b) the reasons why the regulated restrictive practice was used; and (c) the effect on the person's behaviour; and (d) the name of the person who approved the use of the regulated restrictive practice; and (e) the name of the person who applied the regulated restrictive practice or, if seclusion was used, kept the person in seclusion.
