#### 111 Form of oath or affirmation taken or made by witness intermediaries 111 Form of oath or affirmation taken or made by witness intermediaries > The following are prescribed as the form of oath to be taken, or affirmation to be made, respectively, by a witness intermediary for the Act, section 294N(5)— > > > (a) I swear that I will well and faithfully communicate questions and answers and make true explanation of all matters and things as may be required of me according to the best of my skill and understanding, > > > (b) I solemnly and sincerely declare and affirm that I will well and faithfully communicate questions and answers and make true explanation of all matters and things as may be required of me according to the best of my skill and understanding. > > Note. > > A person must either take an oath, or make an affirmation, before acting as a witness intermediary in proceedings to which the Act, Chapter 6, Part 5, Division 1A applies. > > **cl 111:** Am 2023 No 32, Sch 2\[3\] \[7\] \[8\].
