6102 Documents—general heading style
(1) A document for use in relation to a proceeding, and for which there
is an approved form, must be headed in the way set out in the form.
(2) However, a document in a civil proceeding may be headed using an
abbreviation of title sufficient to identify the proceeding.

(3) Subrule (2) does not apply to—
(a) an originating process; or
(b) a document to be served on a person not a party to the

(4) If the parties to a proceeding change, the names of the parties on each
document filed after the change must reflect the state of the parties
after the change.
