#### 72 Matters to be included in application under paragraph 113P(4)(b) of the Act
An application to the Tribunal under paragraph 113P(4)(b) of the Act (to determine a question relating to copying or communicating by a body administering an educational institution) must:
(a) set out the circumstances or events giving rise to the application; and
(b) state whether the applicant is the body or a works collecting society or broadcasts collecting society; and
(c) if the applicant is the body:
(i) identify the nature of education provided by the institution or the nature of the material provided by the institution for the purpose of helping other educational institutions in their teaching purposes; and
(ii) state the name of the collecting society; and
(d) if the applicant is a collecting society:
(i) give details of the notice relating to it under subsection 113V(5) of the Act; and
(ii) state the name of the body; and
(e) state the question; and
(f) request the Tribunal to determine the question.
