146 Inquiry notices—form (1) An inquiry notice must be signed by the police officer giving it. (a) that it is an inquiry notice under this Act; and (b) the financial institution to which the notice applies; and (c) the information that the financial institution is required to give; and (d) how the information is to be given; and (e) that the inquiry notice is a non-disclosable information order. (3) The notice must also include a statement setting out the effect of division 12.6 (Information order offences) in relation to inquiry notices. Monitoring orders Division 12.2
