sec.3 Information for return— Act , s 65
For section 65 (3) (d) of the Act , the following information is prescribed— for a registered worker—the registration number entered for the worker in the register of workers; for a worker other than a registered worker— the worker’s name, date of birth, and residential address; and the worker’s email address and mobile phone number, if any; and the type of community service work performed by the worker for the entity giving the return.
- (a) for a registered worker—the registration number entered for the worker in the register of workers;
- (b) for a worker other than a registered worker— (i) the worker’s name, date of birth, and residential address; and (ii) the worker’s email address and mobile phone number, if any; and (iii) the type of community service work performed by the worker for the entity giving the return.
