Omit “taken.” from regulation 167(1)(c)(iii). Insert instead—

taken;

(d) evidence of the rating given to the service in relation to the National Quality Standard;

(e) details of any penalties imposed on the service;

(f) details of any penalties imposed on persons in relation to their employment or volunteer work at, or other connection with, the service.
