#### 90 Request to approve application of methodology determination to a project with effect from the start of a reporting period For paragraph 128(2)(c) of the Act, a request to approve the application of a specified methodology determination to the project with effect from the start of a reporting period must be accompanied by the following information: (a) the unique project identifier for the project; (b) the title of the applicable methodology determination made under section 106 of the Act for which the approval is being requested and the date the determination commenced; (c) if the applicable methodology determination has been varied under section 114 of the Act—the date the methodology determination was varied; (d) a description of the project; (e) a statement by the applicant that the project meets the requirements of the applicable methodology determination.
