#### 96 Application for variation of accreditation 96 Application for variation of accreditation > An application to an accreditation authority for variation of accreditation must— > > > (a) be made by the holder of the accreditation, and > > > (b) be in a form approved by the accreditation authority, and > > > (c) include or be accompanied by any other information or evidence that the accreditation authority reasonably requires to assess the application, and > > > (d) be accompanied by any fee required, being— > > > > > (i) if the accreditation authority is the Secretary—the amount provided for by Schedule 5, or > > > > > (ii) in any other case—the amount required by the accreditation authority.
