#### 69 Reports of accidents etc. to be supplied to Minister (1) Where: (a) an accident: (i) resulting in loss of human life or serious injury to any person; or (ii) involving a train carrying passengers; or (b) any other serious incident; occurs on a railway owned or operated by the Commission, the Commission shall forthwith furnish to the Minister a report, in writing, of the circumstances of the accident or incident, as the case may be. (2) The Commission shall, as soon as practicable after the expiration of the period of 6 months ending on 30 June, and of the period of 6 months ending on 31 December, in each year, furnish to the Minister a report, in writing, with respect to accidents that have occurred, during the period, on railways owned or operated by it. (3) A report under subsection (2) shall be in such form as the Minister directs by instrument in writing.
