3‑5 Information about the person completing the application and declaration
Information about the individual completing the enrolment application (1) The application must contain the following information relating to the individual completing the application: (a) the individual’s full name; (b) the individual’s job title or position; (c) information on the individual’s role or relationship in relation to the applicant; (d) the individual’s telephone number; (e) the individual’s email address. Declaration by the individual completing the application (2) The application must contain a declaration by the individual completing the application that the information contained in the application is true and correct.
