This Act has been repealed and is no longer in force. It is retained for historical reference.
This instrument sets the daily subsidy rates for Australian Government-funded transition care—short-term (up to 12 weeks) support after hospital discharge—payable to aged care providers: $187.30 basic plus $3.68 extra for recipients with dementia or veterans. It affects transition care providers, older people recovering after hospitalisation, and agencies processing payments, determining funding that helps avoid premature entry into permanent residential aged care and replacing the June 2013 determination.
