101 Improperly witnessing consent
A person must not subscribe his or her name as a witness to the
signature of a person on an instrument of consent unless—
(a) he or she is satisfied that the person who signed the instrument
is a parent or guardian of the child or young person; and
(b) he or she takes reasonable steps to satisfy himself or herself that
the person who signed the instrument understands the effect of
the consent; and
(c) the instrument bears the date on which it is signed by that person.
