Plain English Summary

2/10 complexity

This is a mandatory safety notice for owners and operators of RV-4 model aircraft.

The Problem: The rivets holding the elevator trim tab horn (the part that connects the control mechanism to the small movable surface on the tail) can break or come loose. If this happens, the trim tab could flutter or detach, potentially causing the pilot to lose control of the aircraft.

What You Must Do:

Repeated Inspections: Check the trim tab attachment for loose rivets within 10 hours of flying time after 28 February 1989, and then again every 25 hours of flight time until you complete the permanent fix.
Permanent Modification: Alter the trim tab and horn attachment to a design approved by the Civil Aviation Safety Authority before 28 February 1990.

Who It Affects: Anyone flying or maintaining an RV-4 aircraft.

AD/VG-RV/1 Elevator Trim Tab 3/89 DM

Applicability: All models RV-4.

Requirement: 1. Inspect the elevator trim tab horn attachment to the trim tab for security and loose rivets.

Modify the trim tab horn attachment and the trim tab to a scheme approved by the Authority.

Compliance: 1. Within 10 hours time in service after 28 February 1989 and thereafter every 25 hours time in service until modified.

Prior to 28 February 1990.

Background: A report has been received detailing the failure of two of the four “pop” rivets attaching the horn to the tab. The remaining two rivets were sufficiently loose to allow tab movement. This and/or further loss of attachment integrity could cause flutter and loss of an aircraft.
