What it does
This Airworthiness Directive (AD/OH-58/7, dated 9/2008) requires repetitive inspections of a specified swashplate support assembly on Bell OH-58A (Bell 206A-1) series helicopters, and mandates corrective action where cracking is found. The Directive applies to swashplate support assemblies that are part number 206-010-452-001, -005, or -109 (Applicability). It prescribes visual inspection of the assembly fillet radius area using 10x magnification or greater, with an immediate dye penetrant inspection where a crack is suspected, and replacement of any cracked assembly with a serviceable unit before further flight (Requirement). The Directive identifies a specific serviceable swashplate support assembly part number, 206-010-452-113, the installation of which constitutes terminating action to the Directive’s inspection and re-inspection obligations (Requirement, Note).
The Directive sets initial and recurrent inspection timing. Unless an initial inspection has already been performed, operators must carry out the first inspection within 25 hours time in service after the Directive’s effective date, and thereafter at intervals not to exceed 50 hours time in service, until P/N 206-010-452-113 is installed (Compliance, para 1). For assemblies with less than 1,500 hours time in service, the Directive requires inspection before accumulating 1,525 hours time in service and then at intervals not to exceed 50 hours time in service until the terminating part is installed; for assemblies with 1,500 hours or more it directs the same visual inspection regime (Requirement; Compliance, para 2). The AD became effective 28 August 2008 and was issued under the signature of a CASA delegate, David Villiers, on 14 July 2008 (effective date; signature).
The stated safety rationale is reported in the Directive’s Background: the FAA received reports of cracks in the swashplate support assembly fillet radius near the base, and the specified actions are intended to prevent failure of the swashplate support assembly, loss of control of the main rotor system, and subsequent loss of control of the helicopter (Background). The Directive cross-references BHTI Alert Service Bulletin No. 206-93-74 Revision B (or later FAA approved revision) as the procedural source for the inspections (Requirement). The Directive also notes FAA AD 94-19-02 Amendment 39-9023 as a related instrument (Note).
