The problem: When mechanics tighten the main rotor damper bolt, the surrounding metal can crack around the bolt hole. That's bad news for a helicopter—cracks in rotor components tend to end flights prematurely (and dramatically). Who needs to act: If you own or operate a model 269A, 269A-1, 269B, or early model 269C (serial numbers 0001 through 1165), this applies to you. What you must do: Every time you torque (tighten) that bolt, you must perform a dye penetrant inspection (a chemical process that makes surface cracks visible) on the pitch bearing case. You keep inspecting until you install a spacer under the nut washer—a one-time modification that fixes the underlying issue. The paperwork: Follow Service Information Notice N-189 for the technical details, and ensure compliance after 31 January 1984.

Applicability: All model 269A, 269A-1 and 269B series and 269C serial number 0001 through 1165.

Document: Service information notice N-189.

Compliance: As per requirement document effective after 31 January 1984.

Background: The manufacturer advises, that cracking around the damper attachment bolt hole can occur when torquing the attachment bolt nut, and has consequently introduced a dye penetrant inspection of this area after torque loading, until fitment of a spacer under the nut washer is accomplished.
