What it does
This Civil Aviation Safety Authority airworthiness directive (AD/BELL 212/63 Amdt 1) mandates inspections and corrective work on specified Bell Helicopter Textron Model 212 tail rotor blades to address adhesive-bond voids, attachment screw seating, and aft tip closure rivets. The AD applies to blades with part numbers 212-010-750-009 through -129 installed on Model 212 helicopters, excluding blades whose serial numbers have a prefix of "A" or "AFS" and a discrete list of serial numbers set out in the Applicability block. The AD's corrective actions are expressed as three specific requirements:
- Requirement 1: Inspect the forward tip weight retention block and aft tip closure for adhesive bond voids; remove any tail rotor blade with a void in excess of the limits set in the Component Repair and Overhaul Manual (as stated under Requirement 1).
- Requirement 2: Inspect the forward tip weight retention block attachment countersink screws in four locations to see if each countersink screw head is flush with the abrasion strip surface. If any screw is set below the abrasion strip surface or covered with filler material, install shear pins in accordance with the Accomplishment Instructions, Part A, Shear Pin Installation paragraphs, of Bell ASB 212-00-111 Revision D, or later FAA approved revision (as per Requirement 2 and the reference to the ASB).
- Requirement 3: Install the aft tip closure rivets, re-identify the modified blade by adding the letters "FM", and dynamically balance the tail rotor hub assembly in accordance with the Accomplishment Instructions, Part B, Aft Tip Closure Rivet Installation paragraphs, of Bell ASB 212‑00‑111 Revision D, or later FAA approved revision (Requirement 3).
The AD sets the compliance interval as within 100 hours time in service after 20 December 2007, unless already accomplished. The document notes that Amendment 1 expands the AD’s applicability to include additional part and serial-numbered blades and clarifies the requirement to re-identify the modified blade by adding "FM" and to dynamically balance the tail rotor (under the Amendment 1 note). The AD expressly cross-references FAA AD 2007-22-02 (Amdt 39-15238) and requires following Bell Helicopter Textron Inc. Alert Service Bulletin (ASB) 212-00-111 Revision D, or a later FAA approved revision, as the means of accomplishment.
The AD states the safety rationale in its Background paragraph: CASA received reports of five occurrences in which the forward tip weight retention blocks or aft tip closures were missing, resulting in minor to substantial damage and posing a risk of tail rotor blade loss and consequent loss of control.
