"I am writing to formally resign from my position at Jordan River Health, effective Friday 1 August 2025, in line with my two weeks' notice provided as of 18 July 2025.
This has been an extremely difficult decision for me. I have always taken pride in my role and he work I have contributed, particularly in my time as Practice Leader. However, due to recent events including the handling of my return from personal leave, I feel I am no longer able to continue in my role in a way that supports my wellbeing and values.
I would like to thank team members I've worked closely with and supported during my time here. I remain committed to ensuring a smooth transition and am happy to assist during the handover period to the best of my ability.
Please let me know what steps you require from me to finalise my departure."
We acknowledge receipt of your resignation and confirm that we are exercising our right under Section 19.3 of your Employment Contract to provide you with a payment in lieu of notice for all or part of the notice period.
Your final day of employment with GP Collective will be Monday, 21st July 2025. You are required to attend work on this day to provide a comprehensive handover to the newly appointed Practice Manager. Following this, you will not be required to attend work for the remainder of your notice period.
We thank you for your contribution during your time with us and wish you all the best in your future endeavours."
