### sec.586 Approval of forms
WorkCover’s chief executive officer may approve forms in relation to contracts of insurance for use under this Act.
The Regulator may approve other forms for use under this Act.
Subsection (4) applies if a person—
is required or permitted to do something in an approved form under section 50 , 132 , 133 , 133A , 325B or 542 (the relevant provision ); or
a requirement to send a report or give written notice in an approved form
is required under this Act to make an application for a policy in the approved form (also the relevant provision ).
Without limiting the Electronic Transactions (Queensland) Act 2001 , the person is taken to have complied with the relevant provision when—
the person does the thing by giving the information required on the approved form by phone, or another method, acceptable to the receiver of the approved form (the receiver ); and
if the person’s signature is required on the approved form, the requirement is met under subsection (5) ; and
if the relevant provision requires or permits the approved form to be accompanied by a document when given to the receiver, the person gives the document to the receiver within the reasonable period decided by the receiver.
A requirement that the person sign the approved form as mentioned in subsection (4) (b) is met if—
a method is used to identify the person and to indicate the person’s approval of the information communicated under subsection (4) (a) ; and
having regard to all the relevant circumstances when the method was used, the method was as reliable as was appropriate for the purposes for which the information was communicated; and
the receiver consents to the requirement being met by using the method mentioned in paragraph (a) .

- (c) if the relevant provision requires or permits the approved form to be accompanied by a document when given to the receiver, the person gives the document to the receiver within the reasonable period decided by the receiver.
- (a) a method is used to identify the person and to indicate the person’s approval of the information communicated under subsection (4) (a) ; and
- (b) having regard to all the relevant circumstances when the method was used, the method was as reliable as was appropriate for the purposes for which the information was communicated; and
- (c) the receiver consents to the requirement being met by using the method mentioned in paragraph (a) .
