### sec.213 Accounts for medical treatment, certificate in approved form
This section applies if an insurer is liable for the costs of medical treatment.
Accounts for medical treatment must be sent to the insurer promptly and within 2 months after the treatment is completed.
The accounts must specify—
the worker’s full name, date of birth and residential address; and
any item number that the medical treatment may have that is listed in the relevant table of costs; and
the date of each attendance; and
detailed particulars of treatment; and
the name and place of business of the worker’s employer.
A worker who receives medical treatment must be given a certificate in the approved form free of charge.
(sec.213-ssec.1) This section applies if an insurer is liable for the costs of medical treatment.
(sec.213-ssec.2) Accounts for medical treatment must be sent to the insurer promptly and within 2 months after the treatment is completed.
