### sec.24 Recording of outcome of first assessment
The coordinating practitioner must inform the person of the outcome of the first assessment as soon as practicable after its completion.
Within 2 business days after completing the first assessment, the coordinating practitioner must complete a record of the assessment in the approved form (the first assessment record form ) and give a copy of it to the board.
Maximum penalty—100 penalty units.
The first assessment record form—
must include the outcome of the first assessment, including the coordinating practitioner’s decision in respect of each of the eligibility criteria; and
may be accompanied by documents supporting the coordinating practitioner’s decision in respect of the eligibility criteria.
As soon as practicable after completing the first assessment record form, the coordinating practitioner must give a copy of it, and any documents accompanying it, to the person.
