### sec.657H Application
A person applying for appointment as an account assessor must—
apply in the approved form; and
provide an affidavit demonstrating the person’s eligibility for appointment and stating the following—
any adverse matter known by the person that may affect whether the person is a fit and proper person for appointment;
the hourly rate or rates the person would charge if appointed;
any other matter required by a practice direction to be stated in the affidavit; and
pay the prescribed fee, if any.
The Chief Justice may make a practice direction under this rule requiring other matters relevant to appointment as an account assessor to be stated in the affidavit.
