### sec.115 Making a discipline application
A discipline application must be made in the approved form.
Under section 33 of the Act , the application must state the reasons for the application and be filed.
A discipline application may relate to more than 1 complaint or investigation matter.
A discipline application must state particulars of the alleged unsatisfactory professional conduct or professional misconduct of the person to whom the application relates.
The approved form for subrule (1) must provide for the inclusion of the applicant’s statement of address for service.
This rule applies despite rule 7 .
