### sec.332 Requirements for disclosure
To comply with section 330 or 331 , a person must give the chief executive a disclosure in the approved form.
The information disclosed in the approved form by the person about a conviction or charge of an offence in the person’s criminal history must include—
the existence of the conviction or charge; and
when the offence was committed or alleged to have been committed; and
the details of the offence or alleged offence; and
for a conviction—whether or not a conviction was recorded and the sentence imposed on the person.
(sec.332-ssec.1) To comply with section 330 or 331 , a person must give the chief executive a disclosure in the approved form.
(sec.332-ssec.2) The information disclosed in the approved form by the person about a conviction or charge of an offence in the person’s criminal history must include— the existence of the conviction or charge; and when the offence was committed or alleged to have been committed; and the details of the offence or alleged offence; and for a conviction—whether or not a conviction was recorded and the sentence imposed on the person.
