### sec.123 Records
A responsible person who issues an instrument under this Act must keep a record of the particulars of the instrument at—
if the instrument is issued by the chief executive—an office of the department decided by the chief executive; or
otherwise—an office of the Queensland Police Service decided by the commissioner.
However, a record of particulars of a person’s Queensland driver licence must—
be kept at an office of the department decided by the chief executive; and
include the person’s traffic history.
The person who has custody of the record is—
for a record mentioned in subsection (1) —the person in charge of the office where the record is kept; or
for a record mentioned in subsection (2) —the chief executive.
An extract from or copy of any entry of any particulars of the record that is, or purports to be, certified by the person who has custody of the record as being an extract from or copy of the record, is for a court and all other purposes, evidence of the particulars contained in the record, without requiring the production of the record.
In this section—
instrument means an appointment, approval, authorisation, cancellation, demand, determination, direction, licence, notification, order, suspension or surrender.
issues includes gives or makes.
responsible person means—
the chief executive; or
the commissioner; or
a superintendent.

(sec.123-ssec.2) However, a record of particulars of a person’s Queensland driver licence must— be kept at an office of the department decided by the chief executive; and include the person’s traffic history.
