### sec.60 Procedural requirements for applications to restore registration or registration endorsement
The application must—
be in the approved form; and
be accompanied by—
the fee prescribed under a regulation; and
any documents, identified in the approved form, the board reasonably requires to decide the application.
Information in the application must, if the approved form requires, be verified by a statutory declaration.
