### sec.71 Notification of change in circumstances— Act , s 92 For section 92 (2) of the Act , the following changes in the holder’s circumstances are prescribed— for the holder of a licence— if the licence holder is not a prescribed licensee—a change in the holder’s name; or if the licence is a use licence and the holder is a health practitioner or a veterinary surgeon— a change in the holder’s accreditation, enrolment or registration as a health practitioner or a veterinary surgeon; or a change in a condition attaching to the holder’s accreditation, enrolment or registration; for the holder of an accreditation certificate— a change in the holder’s name; or a change in the holder’s contact details; for the holder of a continuing approval to acquire—a change in the holder’s name; for the holder of a radiation safety officer certificate—a change in the holder’s name. In this section— veterinary surgeon see the Veterinary Surgeons Act 1936 , schedule.
