### sec.103I Requirements for disclosure of changes in criminal history
To comply with section 103H , the person must give the disclosure in the approved form.
The information disclosed by the person about a conviction or charge for an offence in the person’s criminal history must include each of the following—
the existence of the conviction or charge;
when the offence was committed or alleged to have been committed;
details adequate to identify the offence or alleged offence;
for a conviction—
whether or not a conviction was recorded; and
the sentence imposed on the person.

(sec.103I-ssec.1) To comply with section 103H , the person must give the disclosure in the approved form.
(sec.103I-ssec.2) The information disclosed by the person about a conviction or charge for an offence in the person’s criminal history must include each of the following— the existence of the conviction or charge; when the offence was committed or alleged to have been committed; details adequate to identify the offence or alleged offence; for a conviction— whether or not a conviction was recorded; and the sentence imposed on the person.
