### sec.52 Form of key person licence
A key person licence must be in the approved form.
The approved form must provide for the inclusion of the following particulars—
the key person licensee’s name;
a recent photograph of the licensee;
the date of issue of the licence;
whether the licensee is a licensed employee or licensed as a key operator;
the conditions of the licence;
other particulars prescribed under a regulation.
(sec.52-ssec.1) A key person licence must be in the approved form.
(sec.52-ssec.2) The approved form must provide for the inclusion of the following particulars— the key person licensee’s name; a recent photograph of the licensee; the date of issue of the licence; whether the licensee is a licensed employee or licensed as a key operator; the conditions of the licence; other particulars prescribed under a regulation.
