### sec.130 Renewal of licence—application
A licensed supplier may apply for renewal of its supplier’s licence.
An application must—
be made to the commissioner; and
be in the approved form; and
be made—
at least 1 month before the licence expires; or
if, before the licence expires, the commissioner extends the term of the licence—within the extended period.
An application must be accompanied by the following—
a list of the names, addresses and dates of birth of—
the secretary of the applicant; and
the executive officers of the applicant; and
all other persons who have been the secretary or an executive officer of the applicant since the licence was issued or last renewed;
a disclosure affidavit;
Section 182 deals with the requirements for disclosure affidavits.
