### sec.190 Requirements for application
An application for internal review of an original decision must—
be in the approved form; and
for a person who has been given an information notice for the decision—include enough information to enable the chief executive to decide the application; and
be made to the chief executive within—
for a person who has been given an information notice for the decision—28 days after the day the person is given the notice; or
for a person who has not been given an information notice for the decision—28 days after the day the person becomes aware of the decision.
The chief executive may, at any time, extend the period within which the application may be made.
The application does not affect the operation of the original decision or prevent the decision being implemented.

(sec.190-ssec.2) The chief executive may, at any time, extend the period within which the application may be made.
(sec.190-ssec.3) The application does not affect the operation of the original decision or prevent the decision being implemented.
- (a) be in the approved form; and
- (b) for a person who has been given an information notice for the decision—include enough information to enable the chief executive to decide the application; and
- (c) be made to the chief executive within— (i) for a person who has been given an information notice for the decision—28 days after the day the person is given the notice; or (ii) for a person who has not been given an information notice for the decision—28 days after the day the person becomes aware of the decision.
- (i) for a person who has been given an information notice for the decision—28 days after the day the person is given the notice; or
